ABSTRACT

Objective: to analyze, through literature review, the available literature on orthodontic treatment in traumatized permanent teeth. Methods: A literature search was performed in electronic databases (PubMed and SciELO) using the descriptors [tooth injuries] OR [injuries, teeth] OR [injury, teeth] OR [teeth injury] OR [injuries, tooth] OR [injury, tooth] OR [tooth injury] OR [teeth injuries] AND [orthodontics]. Observational studies and clinical trials were included, narrative reviews, laboratory and in vitro studies, case reports and series as well as articles that presented abstracts written in languages other than Portuguese, English and Spanish were excluded from the study. Two reviewers considered the eligibility, the risk of bias of the analyzed data and the qualitative synthesis of the studies included. A total of 1,322 references were found and 4 articles met all inclusion criteria and were included in the qualitative analysis. Some consequences like pulp necrosis and root resorption have been highlighted and trauma severity should be considered when orthodontically intervening in previously traumatized teeth. Final considerations: The traumatized teeth can be orthodontically treated as long as the time of tissue reorganization is respected, and the pull and periodontal conditions are followed up.(AU)


Objetivos: analisar, por meio de revisão de literatura, a respeito do tratamento ortodôntico em dentes permanentes traumatizados. Metodologia: Uma pesquisa bibliográfica foi realizada em bancos de dados eletrônicos (PubMed e SciELO) usando os descritores [tooth injuries] ou [injuries, teeth] ou [injury, teeth] ou [teeth injury] ou [injuries, tooth] ou [injury, tooth] ou [tooth injury] ou [teeth injuries] e [orthodontics]. Foram incluídos estudos observacionais e ensaios clínicos, revisões narrativas, estudos laboratoriais e in vitro, relatos de casos e séries, bem como artigos que apresentassem resumos redigidos em idiomas diferentes do português, inglês e espanhol foram excluídos do estudo. Dois revisores consideraram a elegibilidade, o risco de viés dos dados analisados e a síntese qualitativa dos estudos incluídos. Foram encontradas 1.322 referências e 4 artigos atenderam a todos os critérios de inclusão e foram incluídos na análise qualitativa. Algumas consequências como necrose pulpar e reabsorção radicular têm sido destacadas e a gravidade do trauma deve ser considerada na intervenção ortodôntica em dentes previamente traumatizados. Considerações finais: Os estudos incluídos nesta revisão sugerem que dentes traumatizados podem ser tratados ortodônticamente desde que respeitado o tempo de reorganização tecidual e acompanhadas as condições pulpares e periodontais.(AU)

Introducción: El dolor es el síntoma más común luego de la primera activación de la aparatología ortodóncica. El tratamiento analgésico más utilizado es el paracetamol; sin embargo, su uso inhibe la actividad de la ciclooxigenasa y la síntesis de prostaglandinas, lo que puede afectar el mecanismo del movimiento dental y el remodelado óseo. Objetivo: Evaluar la efectividad del láser de baja potencia para el alivio del dolor en pacientes con tratamiento ortodóntico. Métodos: Se realizó un estudio cuasiexperimental de intervención terapéutica, desde enero hasta diciembre de 2020, en 40 pacientes atendidos en la consulta de ortodoncia del Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba, en quienes se aplicaría fuerza ortodóncica por primera vez. Estos fueron distribuidos de forma aleatoria en 2 grupos de 20 integrantes cada uno: el de estudio, tratado con láser de baja potencia, y el de control, que recibió medicación convencional (paracetamol). Para la validación estadística de la información se emplearon el porcentaje y la prueba de la t de Student para muestras independientes, con 95 % de confiabilidad. Resultados: Luego de tres sesiones terapéuticas, 90,0 % del grupo de estudio refirió no presentar dolor o sentirlo de forma leve, mientras que del grupo de control solo 10,0 % de los pacientes fue ubicado en cualquiera de las dos categorías anteriores. Existió diferencia significativa entre ambos grupos (p=0,00). Conclusiones: Se demostró que el láser, como terapia alternativa, fue más efectivo que el paracetamol.


Introduction: Pain is the most common symptom after the first activation of orthodontic appliance. The most widely used analgesic treatment is paracetamol; however, its use inhibits cyclooxygenase activity and prostaglandin synthesis, which can affect the mechanism of tooth movement and bone remodeling. Objective: To evaluate the effectiveness of low power laser for pain relief in patients with orthodontic treatment. Methods: A quasi-experimental study of therapeutic intervention was carried out, from January to December 2020, in 40 patients treated at the orthodontic clinic of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, in whom orthodontic force was applied for the first time. These were randomly distributed into 2 groups of 20 members each: the study group, treated with low-power laser, and the control group, that received conventional medication (paracetamol). For statistical validation of data, the percentage and the Student's t test for independent samples were used, with 95 % confidence interval. Results: After three therapeutic sessions, 90.0% of the study group reported not presenting pain or feeling it slightly, while only 10.0% of patients in control group were put in any of the two previous categories. There was a significant difference between both groups (p=0.00). Conclusions: Laser, as an alternative therapy, was shown to be more effective than paracetamol.

ABSTRACT

Objective: This study aimed to evaluate the effects of salmon calcitonin administration as a pharmacological anchoring agent in orthodontics and to determine the influence of locally applied calcitonin on serum calcium levels. The secondary aim was to observe the response of dental and periodontal tissues using light microscopy. Methods: Fourteen healthy male adult Wistar rats with an average weight of 250 g had their teeth moved, seven of which received a local injection of salmon calcitonin in the furcation region of the left upper first molar. Concurrently, the remaining seven were used as controls. In the control group, saline solution was injected in the bifurcation region of tooth 26 to subject these animals to the same stress level as those of the experimental group. After 14 days, a 6 mm diameter orthodontic elastic band was inserted between teeth 26 and 27 in all animals to induce the movement of these teeth. The rats were anaesthetised and exsanguinated on day 21. In both groups, tooth movement and serum calcium levels were measured. The jaws were dissected with straight scissors, and tissue blocks containing gingiva, bone and teeth were identified, fixed and demineralised. Then, the pieces were cut into semi?serial slices, stained with hematoxylin, eosin, and Mallory’s trichrome, and analysed under an Axiophot light microscope. Results: There was significantly less tooth movement in the experimental group ( ±X DP ; 0,150 mm ± 0,037) than in the control group (0,236 mm ± 0,044; P = 0,003), while there was no significant difference in serum calcium levels between the two groups (control ±X DP ; 9,53 mg/dl ± 1,53; experimental 10,81 mg/dl ± 1,47; P = 0,15). Conclusion: While calcitonin did not completely inhibit osteoclast activity, it promoted orthodontic anchorage, apparently, by local action.

ABSTRACT

Abstract The aim of this study was to quantify the force exerted by tandem archwires in a specific system of passive self-ligating bracket. Forty-eight thermo-activated nickel-titanium orthodontic archwires were separated into four groups (n = 12): G1 - two .014" + .014" round archwires; G2 - two .014" + .016" round archwires; G3 - .014" x .025" rectangular archwire; and. G4 - .016" x .022" rectangular archwire. Brackets were fixed onto teeth 1.5 to 2.5 using a device that represented the upper teeth, maintaining an interbracket distance of 6.0 mm. The deflection tests were performed using the structure representative of tooth 1.1 as support on the Instron testing machine at a speed of 2.0 mm/min. The archwires were evaluated at deflections of 0.5 mm, 1.0 mm, and 1.5 mm. The data were analyzed by a generalized linear model, considering values at different deflections as repeated measurements in the same experimental unit (α = 0.5%). At 0.5 mm, higher forces were observed in G2 and G3, which did not differ significantly (p > 0.05). The lowest force was observed in G4 (p < 0.05). At 1.0 mm and 1.5 mm, the highest force was observed in G3, followed by G4 and G2 (p < 0.05). The lowest force was observed in G1 (p < 0.05). In general, tandem archwires (same or different calibers) in a specific passive self-ligating bracket exerted lower force when compared with rectangular archwires.

ABSTRACT Introduction: Bisphosphonates have an inhibitory impact on osteoclastic activity, reducing bone resorption. However, the influence of risedronate on tooth movement is not well-defined. Objective: This systematic review assessed the effect of risedronate intake on orthodontic tooth movement. A case report was also provided. Methods: Two independent reviewers searched six databases (PubMed, Web of Science, Ovid, Lilacs, Scopus, and Open Grey). The searches were carried out in April/2020, and an update was set in place in June/2023. Therefore, the searches considered a timeline from the databases' inception date until June/2023, with no publication date and/or language restrictions. The clinical question focused on evaluating the orthodontic tooth movement and relapse movement (Outcome) in animals (Population) exposed to risedronate (Exposure), compared to control groups (Comparison). The Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines were applied, and the protocol was registered in PROSPERO (CRD42020168581). The risk of bias was determined using the Systematic Review Centre for Laboratory Animal Experimentation protocol (SYRCLE). Results: Two studies in rats and one in guinea pigs were included in the systematic review. The studies reported a decrease in orthodontic tooth movement, a reduction in the relapse movement, and a reduced number of positive tartrate-resistant acid phosphatase (TRAP) cells, with a significantly reduced number of bone gaps after the administration of risedronate in rats. A case report illustrated the effects of risedronate administration in one patient. Conclusion: Based on the systematic review, risedronate seems to impair orthodontic tooth movement and relapse due to a decrease in bone resorption cells.


RESUMO Introdução: Os bifosfonatos têm um impacto inibitório na atividade osteoclástica, reduzindo a reabsorção óssea. No entanto, a influência do risedronato no movimento dentário não está bem definida. Objetivo: Esta revisão sistemática avaliou o efeito do uso de risedronato no movimento ortodôntico dos dentes. Um relato de caso também é apresentado. Métodos: Dois revisores independentes pesquisaram seis bases de dados (PubMed, Web of Science, Ovid, Lilacs, Scopus e Open Grey), considerando o período de abril de 2020 até junho de 2023, sem restrições de data e/ou idioma de publicação. A questão clínica focou em avaliar o movimento ortodôntico dos dentes e movimento de recidiva (resultado) em animais (população) expostos ao risedronato (exposição) em comparação com grupos de controle (comparação). Foram aplicadas as Diretrizes Preferenciais para Revisão Sistemática e Metanálise (PRISMA) e um protocolo foi registrado no PROSPERO (CRD42020168581). O risco de viés foi determinado utilizando o protocolo do Centro de Revisão Sistemática para Experimentação em Animais de Laboratório (SYRCLE). Resultados: Dois estudos em ratos e um em porquinhos-da-índia foram incluídos na revisão sistemática. Os estudos relataram uma diminuição no movimento ortodôntico dos dentes, uma redução no movimento de recidiva e um número reduzido de células positivas à fosfatase ácida tartarato-resistente (TRAP) com um número significativamente reduzido de falhas ósseas após a administração de risedronato em ratos. Um relato de caso ilustrou os efeitos da administração de risedronato em uma paciente. Conclusão: Com base na revisão sistemática, o risedronato parece interferir no movimento ortodôntico dos dentes e na recidiva devido a uma diminuição nas células de reabsorção óssea.

ABSTRACT Objective: To investigate the effects of orthodontic tooth movement on clinical attachment level (CAL) changes in treated periodontitis in adult patients with malocclusion. Material and Methods: Present study is based on PRISMA guidelines; all articles published in international databases such as PubMed, Scopus, Science Direct, and Embase between 2012 to May 2022 are included. 95% confidence interval (CI) for mean difference with fixed effect modal and inverse-variance were calculated. Data analysis was performed using STATA.V16 software. Results: In the initial review, duplicate studies were eliminated, abstracts of 175 studies were reviewed, two authors reviewed the full text of 21 studies, and finally, eleven studies were selected. The mean of CAL gain was 2.29 mm (MD, 95% CI -2.47 mm, -2.12 mm; p=0.00) (I2=91.81%; p=0.00; high heterogeneity). The mean difference of PPD changes was -1.93 mm (MD, 95% CI -2.07 mm, -1.80 mm; p=0.00) (I2=98.52%; p=0.00; high heterogeneity). Conclusion: Due to the limitations of the study and based on the meta-analysis, it is observed that orthodontic treatment is performed with higher success after reconstructive surgery with periodontal improvement.

RESUMEN Introducción: El tratamiento ortodóncico quirúrgico puede ser una terapia de elección para los caninos impactados, ya que estos deben conservarse en las arcadas por sus beneficios estéticos y funcionales. La inclusión bilateral de estos en el maxilar es menos frecuente y su tratamiento más complejo. Objetivo: Describir los resultados del tratamiento ortodóncico quirúrgico, en una paciente con inclusión bilateral de caninos superiores. Caso clínico: Paciente femenina de 16 años con antecedentes de salud, que acudió a consulta de Estomatología General Integral en compañía de la madre, preocupada por el aspecto de sus colmillos más blancos y pequeños. Al examen bucal, se observó una completa dentición permanente, excepto los caninos que, en su lugar, se encontraban los dientes 53 y 63. Presentó relieve anormal de la zona anterior de la mucosa palatina, con ligero aumento de volumen a la palpación. Se realizó radiografía panorámica y periapicales y se confirmó la presencia de ambos dientes impactados. Se realizó tratamiento ortodóncico quirúrgico. Conclusiones: El tratamiento ortodóncico quirúrgico devolvió a la paciente la función y la estética, mediante la correcta ubicación de ambos caninos en las arcadas.


ABSTRACT Introduction: Orthodontic surgical treatment could be a therapy of choice for the impacted canines, as they should be kept in the dental arches for their functional and aesthetic benefits. The bilateral impaction of these in the maxillary bone is less frequent and their treatment is more complex. Objective: To describe the results of the orthodontic surgical treatment in a female patient with bilateral impaction of upper canines. Case report: A 16-year-old female patient with a health history was assisted by a general dentist in the company of the mother, because she was worried about the aspect of her cupids which were whiter and littler. The oral examination showed the presence of a permanent and complete dentition with the exception of the upper canines, in their place, were teeth number 53 and 63. It was also found an abnormal relief in the anterior zone of the hard palate with a slight increase of volume at palpation. It was taken panoramic and periapical X-ray and it was confirmed the presence of both impacted teeth. It was made orthodontic surgical treatment. Conclusions: Orthodontic surgical treatment gave back the patient the function and aesthetic by means of the correct location of both teeth in the dental arches.

RESUMEN Introducción: Si un canino impactado cruza la línea media desde su posición normal al lado contrario, se denomina transmigración. Esta presenta bajos rangos de prevalencia por lo que se le considera una condición rara. El tratamiento ortodóncico quirúrgico es una de sus opciones terapéuticas. Objetivo: Describir los resultados del tratamientoortodóncico quirúrgico en un pacientecon canino inferior derecho transmigrado. Caso clínico: Paciente masculino de 11 años de edad con antecedentes de salud que acudió a consulta de Estomatología General Integral por tener los dientes botados y por la falta de uno que mudó y el sucesor aún no ha brotado. El caso se refiere al servicio de Ortodoncia. Al examen bucal destacó la ausencia clínica del 43, sin espacio necesario para su ubicación en la arcada. El paciente clasificó como síndrome de clase II división 1 de Moyers. Se indicó estudio radiográfico y se confirmó el diagnóstico de canino incluido, transmigrado y mesioangular en región mentoniana. Se decidió realizar tratamiento ortodóncico quirúrgico para colocar el canino en su posición. Conclusiones: El tratamiento ortodóncico quirúrgico devolvió al paciente la función y la estética, mediante la correcta ubicación del canino en la arcada. Este puede ser considerado como una alternativa terapéutica en el caso de canino mandibular transmigrado que presenta patrón tipo 1.


ABSTRACT Introduction: If an impacted canine tooth crosses the midline towards the opposite side, it is considered as dental transmigration. It is a rare condition with low ranges of prevalence. The surgical-orthodontic treatment is one of the therapeutic options. Objective: To describe the orthodontic-surgical treatment results, in a patient, with transmigrated lower right canine. Case report: An 11-year-old male patient attended the general practitioner for dental evaluation, his chief complaint was the absence of one deciduous tooth and the permanent still un-erupted and the tipping of the upper incisors. During the orthodontic examination was noted insufficient space to tooth 43 in the arch. The patient was diagnosed with Moyers' class II malocclusion division 1 syndrome. The radiographic examination confirmed the diagnosis of impacted and transmigrated lower right canine, mesioangular position across the midline within the chin area. Orthodontic-surgical treatment was carried out to bring the canine tooth to its functional position. Conclusions: Orthodontic-surgical treatment restored the function and aesthetics to the patient, through the right positioning of the canine in the arch. It could be considered as a therapeutic alternative in the case of transmigrated mandibular canine with type 1 pattern.

RESUMEN: En pacientes con periodontitis, la migración dentaria patológica es una consecuencia de la pérdida de soporte periodontal. El siguiente reporte de caso aborda el tratamiento periodontal y ortodóncico de una paciente con periodontitis y migración dentaria patológica. El protocolo utilizado fue el protocolo de la Unidad de Ortodoncia y Periodoncia de la Facultad de Odontología de la Universidad de Chile. La estabilidad periodontal se ha mantenido por al menos 6 años luego de finalizado el tratamiento de ortodoncia logran-do funcionalidad y estética de la dentición natural.


ABSTRACT: In patients with periodontitis, pathologic tooth migration is a consequence of loss of periodontal support. The following case report addresses the periodontal and orthodontic treatment of a patient with periodontitis and pathologic tooth migration. The protocol used was the protocol of the Orthodontics and Periodontics Unit of the Faculty of Dentistry of the University of Chile. The periodontal stability has been maintained for at least 6 years after the end of orthodontic treatment, achieving functionality and aesthetics of the natural dentition.

ABSTRACT

Abstract Three-dimensionally evaluation of the treatment changes of a Herbst appliance using a lower anchorage unit not touching the lingual surface of the lower incisors. The sample consisted of 23 Class II:1 patients (12 males, 11 females) with a mean age of 15.7±1.7 years treated with a Flip-Lock Herbst® appliance (TP Orthodontics, Inc., La Porte, IN, USA). The lower anchorage unit for the Herbst appliance consisted of two anchor bands connected by a lingual arch with 3mm distance from the incisor's lingual surface. Treatment changes in mandibular incisor inclination, overjet and overbite were evaluated by means of cone beam computed tomography images (i-CAT® Classic unit, Imaging Sciences International, Hatfield, PA, USA) obtained before and after treatment with the Herbst appliance. On average, there was a statistically significant increase in mandibular incisor inclination (2.6+1.8°) and a reduction in overjet (3.2+2.2mm) and overbite (1.3+0.9mm). Genders did not differ significantly. Incisor proclination was however only seen in 74% of the patients. The changes in mandibular incisor inclination were associated with the changes in overjet (/r/ = 0.1 to 0.5) and overbite (/r/ = 0.3 to 0.7). A Herbst appliance with a mandibular anchorage unit distant from the incisor's lingual surface results in smaller amounts of mandibular incisor proclination compared to literature. However, as it induces canine anchorage loss, the decreased amount of proclination may not prevail after multibracket treatment.(AU)


Resumo Avaliação tridimensional das alterações induzidas pelo aparelho Herbst utilizando a unidade de ancoragem inferior afastada da superfície lingual dos incisivos. A amostra incluiu 23 pacientes Classe II:1 (12 masculino, 11 feminino), média de idade 15,7 ± 1,7, tratados com aparelho Herbst Flip-Lock® (TP Orthodontics, Inc., La Porte, IN, EUA). A unidade de ancoragem inferior do aparelho Herbst consistiu-se de duas bandas conectadas por um arco lingual afastado 3mm da superfície lingual do incisivo. As alterações induzidas pelo tratamento na inclinação dos incisivos inferiores, trespasse horizontal e trespasse vertical foram avaliadas por meio de imagens de tomografias computadorizadas de feixe cônico (i-CAT® Classic unit, Imaging Sciences International, Hatfield, PA, USA) obtidas antes e após o tratamento com aparelho Herbst. Na média, houve diferença significativa com aumento da inclinação dos incisivos inferiores (2,6+1,8°) e diminuição do trespasse horizontal (3,2+2,2mm) e do trespasse vertical (1,3+0,9mm). Não houve diferença estatística entre os sexos. No entanto, a vestibularização do incisivo ocorreu em apenas 74% dos pacientes. As alterações na inclinação dos incisivos inferiores apresentam correlação estatisticamente significativa com as alterações no trepasse horizontal (/r/ = 0,1 a 0,5) e no trespasse vertical (/r/ = 0,3 a 0,7). O aparelho Herbst com uma unidade de ancoragem inferior afastada da superfície lingual dos incisivos resulta em menor quantidade de vestibularização do incisivo inferior em comparação com a literatura. Entretanto, como isto induz perda de ancoragem do canino, a diminuição da vestibularização pode não prevalecer ao final tratamento ortodôntico com braquetes. (AU)

ABSTRACT

Extrusive tooth movements are an important resource in orthodontic treatment and allow the manipulation of teeth and periodontal tissues. They can be performed quickly or slowly, depending on the patient's need. Rapid extrusion is indicated for cases in need of prosthetic preparation or restoration, where the bone and gingival tissues are intact, such as horizontal and oblique fractures, coronary or external root resorption, iatrogenic perforations (trepanations) and the presence of subgingival caries. The aim of this study is to describe the treatment of a patient who had a coronary fracture of the right upper central incisor, with a limit located 1 mm above the level of the bone crest. Rapid orthodontic extrusion was performed, to restore biologic distances and allow the preparation for prosthesis. It started with partial differentiated bonding of a fixed orthodontic appliance to the upper arch, to allow for a 3mm orthodontic extrusion. At the end of the extrusion, periodontal surgery was performed to increase the clinical crown and endodontic treatment. After these procedures, the case was concluded with the fixed prosthesis installation. The realization of an integrated planning allowed the restoration of aesthetics, with preservation of the functional periodontal limits for the patient.(Au)


Os movimentos dentários extrusivos constituem um recurso importante no tratamento ortodôntico e permitem a manipulação dos dentes e dos tecidos periodontais. Eles podem ser realizados de forma rápida ou lenta, dependendo da necessidade do paciente. A extrusão rápida está indicada para casos com necessidade de preparo protético ou restauração, onde os tecidos ósseo e gengival encontram-se íntegros, tais como fraturas horizontais e oblíquas, reabsorções coronárias ou radiculares externas, perfurações iatrogências (trepanações) e presença de cárie subgengival. O objetivo deste trabalho é descrever o tratamento de uma paciente que apresentava fratura coronária do incisivo central superior direito, com limite localizado 1mm acima do nível da crista óssea. Realizou se extrusão ortodôntica rápida, com a finalidade de restabelecer as distâncias biológicas e permitir o preparo para prótese. Iniciou-se com colagem diferenciada parcial de aparelho ortodôntico fixo no arco superior, para permitir a extrusão ortodôntica de 3mm. Ao término da extrusão, realizou-se cirurgia periodontal para aumento da coroa clínica e tratamento endodôntico. Após esses procedimentos, o caso foi finalizado com a instalação da prótese fixa. A realização de um planejamento integrado permitiu o restabelecimento da estética, com preservação dos limites periodontais funcionais para a paciente. (Au)

ABSTRACT

Abstract The purpose of this in vitro study was to evaluate the force decrease of different elastomeric chains after different times: initial, 10 minutes, 1 day, 28 days and after mechanical brushing. Twenty orthodontic elastomeric chains segments were utilized for each commercial brand. Initially, the elastomeric chain of 15mm long were immediate stretched up to 20 mm in an Instron and the force was measured in gf. After all specimens were placed stretched on rectangular acrylic jigs with distance of 20 mm, immersed in deionized water at 37oC for 10 minutes and the force (gf) was measured again. Five test measurements of remaining force were made at the following time intervals: initial, 10 minutes, 1 day, 28 days and mechanical brushing. After 28 days, the acrylic plates with the specimens were adapted in the mechanical brushing machines (MSCT 3) and the elastomeric chains were submitted to mechanical brushing and the force (gf) measure again. The force (gf) was submitted to mixed-model ANOVA and Sidak post-hoc test (α=0.05). A statistically significant reduction in the force was found for all orthodontic elastomeric chain types after 1 day, 28 days and mechanical brushing (p<0.05). Morelli and 3M Unitek elastomeric chains showed significantly higher force than Abzil and GAC (p<0.05) after 1 day, 28 days and mechanical brushing. In conclusion, the force delivered by all elastomeric chains decayed rapidly over time. Morelli and 3M Unitek elastomeric chains consistently had a significantly greater force after mechanic brushing, while GAC the lowest.


Resumo O objetivo deste estudo in vitro foi avaliar a redução da força de diferentes cadeias elastoméricas após diferentes tempos: inicial, 10 minutos, 1 dia, 28 dias e após escovação mecânica. Vinte segmentos de cadeias elastoméricas ortodônticas foram utilizadas para cada marca comercial. Inicialmente, a cadeia elastomérica de 15 mm de comprimento foi esticada imediatamente até 20 mm na Instron e a força foi medida em (gf). Após, todas as amostras foram adaptadas esticadas em placas retangulares de acrílico na distância de 20 mm, imersas em água deionizada a 37o C por 10 minutos e a força (gf) foi medida novamente. Cinco medidas de força foram feitas nos seguintes intervalos de tempo: inicial, 10 minutos, 1 dia, 28 dias e após a escovação mecânica. Após 28 dias, as placas de acrílico com as amostras foram adaptadas na máquina de escovação (MSCT 3) e as cadeias elastoméricas foram submetidas a escovação mecânica e a medida de força (gf) novamente. Os dados da força (gf) foram submetidos a ANOVA modelo misto e teste post-hoc de Sidak (α = 0,05). Uma redução estatisticamente significativa na força foi encontrada para todos os tipos de cadeia elastomérica ortodôntica após 1 dia, 28 dias e escovação mecânica (p<0,05). As cadeias elastoméricas Morelli e 3M Unitek apresentaram força significativamente superior em relação a Abzil e GAC (p<0,05) após 1 dia, 28 dias e escovação mecânica. Em conclusão, a força distribuída por todas as cadeias elastoméricas decaiu rapidamente com o tempo. As cadeias elastoméricas Morelli e 3M Unitek consistentemente tiveram uma força significativamente maior após a escovação mecânica, enquanto GAC a mais baixa.

Introducción: La falta de un incisivo permanente no solo genera un efecto adverso en la estética facial, también altera la función, en especial, la guía incisiva. Los incisivos superiores pueden sufrir bloqueo mecánico o cambio en su erupción por un diente supernumerario, un golpe u otro factor. El tratamiento de elección es el quirúrgico-ortodóncico. El pronóstico dependerá de la edad, situación, posición del diente, morfología, tamaño, maduración radicular y método de tracción. Conocer el uso de una aparatología ortodóncica que sea fácil de manipular y pueda ser usada desde edades tempranas servirá de valioso aporte. Objetivo: Mostrar el uso exitoso de un cantiléver para traccionar ortodóncicamente un incisivo impactado en posición horizontal. Presentación del caso: Paciente de 8 años con maloclusión clase I, pieza 2.1 retenida en posición horizontal, presencia de supernumerario y persistencia de pieza 6.1. Se opta por extracción de supernumerario, liberación de pieza 2.1 y tracción ortodóncica. Se usó un cantiléver por vestibular confeccionado en arco de acero redondo 0.020" con dos círculos en cada extremo para brindar elasticidad y anclaje. La fuerza utilizada fue de 70 g, pasado 6 meses se alcanzó el plano de oclusión. Se cementaron brackets y tubos, se continuó con la secuencia de arcos, hasta llegar al arco acero 0.021"x0.025" durante 11 meses. Se obtiene una posición final óptima que favorece la formación radicular y cierre apical. Conclusiones: El uso del cantiléver para el tratamiento ortodóncico de incisivos permanentes impactados en posición horizontal demostró ser exitoso, fácil de manipular y controlar(AU)


Introduction: The lack of a permanent incisor not only generates an adverse effect on facial aesthetics but also alters its function, especially the incisor guidance. Upper incisors can suffer mechanical blockage or change in their eruption due to a supernumerary tooth, a blow or another factor. The treatment of choice is orthodontic-surgical. The prognosis depends on the age, tooth position, morphology, size, root maturation and traction method. Knowing the use of an orthodontic appliance, which is easy to handle and can be used from an early age, will be of valuable contribution. Objective: To show the successful use of a cantilever to enable orthodontic traction of an impacted incisor in a horizontal position. Case presentation: Eight-year-old patient with class I malocclusion, specimen 2.1 retained in a horizontal position, presence of supernumerary tooth and persistence of specimen 6.1. Extraction of the supernumerary, release of specimen 2.1 and orthodontic traction is chosen. A buccal cantilever made of a 0.020" round steel arch with two circles at each end was used to provide elasticity and anchoring. The force used was 70 g. Six months after, the occlusion plane was reached. Brackets and tubes were cemented and the sequence of arches was continued until the 0.021"x0.025" steel arch was reached in 11 months. An optimal final position is obtained, favoring root formation and apical closure. Conclusions: The use of the cantilever for orthodontic treatment of impacted permanent incisors in a horizontal position proved to be successful as well as easy to manipulate and control(AU)

ABSTRACT

A luxação extrusiva é caracterizada pela separação parcial ou total entre raiz e ligamento periodontal (LPD), podendo causar danos ao cemento por meio da compressão radicular contra o osso alveolar, além da ruptura do feixe vásculonervoso apical. A movimentação dentária induzida (MDI) é uma resposta de caráter inflamatório do conjunto LPD e osso alveolar, mesmas estruturas envolvidas na luxação extrusiva, a um estímulo provocado por uma força ortodôntica. Uma informação escassa na literatura é o efeito provocado pela movimentação dentária induzida em dentes que foram submetidos à luxação extrusiva (LE). O objetivo deste trabalho foi avaliar o efeito da movimentação dentária induzida sobre dentes submetidos à luxação extrusiva. Para a realização deste pesquisa, foram utilizados 40 ratos machos Wistar divididos em 4 grupos: o grupo controle (GC); o grupo Luxação Extrusiva (LE); o grupo Movimentação Dentária Induzida (MDI); e o grupo LEMDI. A LE foi exercida por uma força de 1.500Cn e a MDI por uma força de 50Cn por um período de 7 dias, após o qual os animais foram eutanasiados, as peças submetidas ao processamento laboratorial e obtidas lâminas que foram coradas por meio de hematoxilina e eosina. Foi realizada uma análise descritiva considerando-se quatro áreas ao redor da raiz mésio-vestibular: A1 região da crista óssea mesial, A2 região médio- cervical da face mesial, A3 região médio-apical da face distal e A4 região cervical da face. Em cada uma destas áreas foram observados os evento: reabsorção óssea frontal e à distância, reabsorção radicular, infiltrado infamatório e hemoragia no LPD.Os resultados mostraram que a associação entre MDI e LE provocou grande destruição óssea na crista óssea na A1, moderadas reabsorções radiculares na A3, ausência de infiltrado inflamatório em todas as áreas e hemorragia exuberante do LPD nas áreas 2 e 3. Nos demais grupos, tais observações foram ausentes ou quando presentes consideradas muito discretas. Conclui-se que, uma vez que a MDI em dentes submetidos a LE aumenta as áreas de hemorragia no LPD, provoca grande perda óssea frontal na crista alveolar, a MDI deveria ser interrompida, aguardando-se o período de reparo do LPD(AU)


Extrusive dislocation is characterized by partial or complete separation of the root and periodontal ligament (PDL), which can cause damage to the cementum by compressing the root against the alveolar bone, and rupture of the apical nervevascular bundle. Induced tooth movement (ITM) is an inflammatory response of the whole of the SMP and alveolar bone, the same structures involved in extrusive dislocation, to a stimulus caused by an orthodontic force. Little information is available in the literature on the effect caused by induced tooth movement in teeth that underwent extrusive luxation (EL). The aim of this study was to evaluate the effect of induced tooth movement on teeth submitted to extrusive dislocation. For this research 40 male Wistar rats were used, divided into 4 groups: The control group (CG); the Extrusive luxation group (EL); the Induced Tooth Movement group (ITM); and the LEMDI group. The LE was exerted by a force of 1,500Cn and the ITM by a force of 50Cn for a period of 7 days, after which the animals were euthanized, the specimens were submitted to laboratory processing and slides were obtained and stained using hematoxylin and eosin. A descriptive analysis was performed considering four areas around the mesial vestibular root: A1 mesial bone crest region, A2 mesial face midcervical region, A3 distal face mid-apical region and A4 face cervical region. In each of these areas the following events were observed: frontal and distal bone resorption, root resorption, infamatory infiltrate and hemorrhagia in the PDL. The results showed that the association between MDI and LE caused great bone destruction in the bone crest in A1, moderate root resorption in A3, absence of inflammatory infiltrate in all areas, and exuberant hemorrhage of the PDL in areas 2 and 3. In the other groups, such observations were absent or when present considered very discrete. We conclude that, since ITM in teeth submitted to EL increases the areas of hemorrhage in the PDL, causes great frontal bone loss in the alveolar ridge, ITM should be interrupted, waiting for the period of repair of the PDL(AU)

Abstract Background: IGF-1 may be an important factor in bone remodeling, but its mechanism of action on osteoclasts during orthodontic tooth movement is complex and unclear. Methodology: The closed-coil spring was placed between the left maxillary first molar and upper incisors with a force of 50 g to establish an orthodontic movement model. Eighty SD rats were randomized to receive phosphate buffer saline or 400 ng rhIGF-1 in the lateral buccal mucosa of the left maxillary first molar every two days. Tissue sections were stained for tartrate-resistant acidic phosphatase (TRAP), the number of TRAP-positive cells was estimated and tooth movement measured. Results: The rhIGF-1 group exhibited evidential bone resorption and lacuna appeared on the alveolar bone compared to the control group. Moreover, the number of osteoclasts in compression side of the periodontal ligament in the rhIGF-1 group peaked at day 4 (11.37±0.95 compared to 5.28±0.47 in the control group) after the orthodontic force was applied and was significantly higher than that of the control group (p<0.01). Furthermore, the distance of tooth movement in the rhIGF-1 group was significantly larger than that of the control group from day 4 to day 14 (p<0.01), suggesting that rhIGF-1 accelerated orthodontic tooth movement. Conclusion: Our study has showed that rhIGF-1 could stimulate the formation of osteoclasts in the periodontal ligament, and accelerate bone remodeling and orthodontic tooth movement.

ABSTRACT Introduction: Orthodontic retraction of the anterior teeth is indicated when the patient has a malocclusion with protrusion of the incisors. Several mechanics are indicated to perform this retraction. Objective: The objective of this study was to compare the strains generated by four different types of retraction mechanics along the roots of the anterior teeth. Methods: A photoelastic model simulating an arch with first premolars extraction was made. Sixty retraction archwires were prepared, including fifteen for each type of mechanics evaluated: sliding, teardrop loop spring, T-loop spring and double key loop archwire. The strains were observed in two perspectives: occlusal and oblique. In the occlusal perspective, strains were compared among the six anterior teeth. From the oblique perspective, strains were compared among the thirds of the left canine root. Results: In the occlusal perspective, the teardrop loop spring mechanics presented greater strains, followed by T-loop spring, double key loop archwire and sliding mechanics. In all mechanics, strains were more concentrated in the canines than in the incisors. From the oblique perspective, the teardrop loop mechanics generated greater strains in the cervical regions of the canine, and in the apical regions, no differences were found in strains among the four types of mechanics. In the same mechanics, greater strains were present in the cervical zones. Conclusion: The teardrop loop spring retraction mechanic presented the greatest mean strain, and the sliding retraction mechanic presented the lowest mean strain on the root of anteroinferior teeth in the occlusal and oblique perspectives.


RESUMO Introdução: A retração ortodôntica dos dentes anteriores é indicada quando o paciente apresenta má oclusão com vestibularização dos incisivos. Diferentes mecânicas são indicadas para realizar essa retração. Objetivo: O objetivo do presente estudo foi comparar as tensões geradas por quatro diferentes tipos de mecânicas de retração nas raízes dos dentes anteriores. Métodos: Um modelo fotoelástico foi confeccionado simulando uma arcada com os primeiros pré-molares extraídos. Foram preparados 60 arcos de retração, sendo quinze para cada tipo de mecânica avaliada: deslizamento, alça de Bull, alça em T e arco de dupla chave. As tensões foram observadas em duas perspectivas: oclusal e oblíqua. Na vista oclusal, as tensões foram comparadas entre os seis dentes anteriores. Na vista oblíqua, as tensões foram comparadas entre os terços radiculares do canino esquerdo. Resultados: Na vista oclusal, a mecânica com alça de Bull apresentou maiores tensões, seguida da alça em T, arco de dupla chave e mecânica de deslizamento. Em todas as mecânicas, as tensões se concentraram mais nos caninos do que nos incisivos. Na vista oblíqua, a mecânica com alça de Bull gerou maiores tensões nas regiões cervicais dos caninos; nas regiões apicais, não houve diferenças nas tensões entre os quatro tipos de mecânicas. Dentro de uma mesma mecânica, as maiores tensões estiveram presentes nas regiões cervicais. Conclusão: A mecânica de retração com alça de Bull apresentou a maior média de tensões, e a mecânica de retração por deslizamento apresentou a menor média de tensões na raiz dos dentes anteroinferiores, nas vistas oclusal e oblíqua.

ABSTRACT Objective: The objectives of the present study were to develop a method for longitudinally measuring tooth rotation, inclination and angulation on digital models, and to test the method validity and reliability. Methods: The initial and final planned models of 14 patients treated with Invisalign® (386 teeth) were exported from ClinCheck®. The rotation, inclination and angulation values were assessed for the incisors, canines, premolars and molars, in both models, using trigonometry. An application was developed in Python 2.7 to automate the measurements. The ∆planned (variation in the position between the initial and final planned models) was obtained for each tooth and each type of movement. To test the validity, the degree of agreement between the ∆planned and the values available in the Invisalign® Table of Movements was assessed using the Intraclass Correlation Coefficient (ICC) and Bland-Altman analysis. For intra and inter-rater reliabilities, the ∆planned was obtained again. Results: Excellent ICCs (> 0.9) and limits of agreement with narrow and clinically acceptable discrepancies were obtained for the rotation of all teeth (except maxillary canines, which had broader limits: -3.47 - 5.43) and for the inclination of premolars and molars. The inclination of anterior teeth and angulation of all teeth had ICCs and limits that were not indicative of great agreement. The reliability was high for the three movements (discrepancy <2°). Conclusions: The method developed is reliable and suitable for longitudinally measuring inclination (posterior teeth) and rotation (except maxillary canines). It has limited value for the other movements measurements.


RESUMO Objetivo: Os objetivos do presente estudo foram desenvolver um método para medir longitudinalmente a rotação, inclinação e angulação dentárias em modelos digitais, e testar sua validade e reprodutibilidade. Material e Métodos: Os modelos inicial e final planejado de 14 pacientes tratados com Invisalign® (386 dentes) foram exportados do ClinCheck®. Os valores de rotação, inclinação e angulação foram calculados para incisivos, caninos, pré-molares e molares, em ambos os modelos, por meio de trigonometria. Um aplicativo desenvolvido em Python 2.7 foi utilizado para automatização das medidas. O ∆planejado (variação na posição entre os modelos inicial e final planejado) foi obtido para os movimentos descritos, nos diferentes grupos de dentes. A validade do método foi verificada pelo grau de concordância entre o ∆planejado e os valores disponíveis na Tabela de Movimentos do Invisalign®, utilizando-se o Coeficiente de Correlação Intraclasse (ICC) e a análise de Bland-Altman. O ∆planejado foi novamente calculado para análise da reprodutibilidade intra e interexaminadores. Resultados: Excelentes ICCs (> 0,9) e limites de concordância com discrepâncias reduzidas e clinicamente aceitáveis foram obtidos para a rotação de todos os dentes (exceto os caninos superiores, que expressaram limites mais amplos: -3,47 - 5,43) e para a inclinação de pré-molares e molares. A inclinação dos dentes anteriores e a angulação de todos os dentes apresentaram ICCs e limites não representativos de boa concordância. A reprodutibilidade foi alta para os três movimentos (discrepância < 2°). Conclusões: O método desenvolvido é reprodutível e se mostrou adequado para mensuração longitudinal da inclinação de dentes posteriores e rotação de todos os dentes, exceto caninos superiores. Apresenta valor limitado para as demais mensurações.

ABSTRACT With improvements in dental aesthetic requirements an increasing number of adults are seeking orthodontic treatment that, along with current lifestyle and eating habits of the adult population, makes orthodontists more likely to encounter patients with metabolic disorders such as diabetes mellitus. Speculated that the diabetic patient during orthodontic treatment may not experience a physiological healing process as a healthy patient. Therefore, the objective of this work is to present a current and contextualized review of the mechanisms by which uncontrolled diabetes mellitus impacts on bone remodeling and orthodontic tooth movement during the application of orthodontic forces. The following databases were searched MEDLINE (via PubMed), Scopus, Web of Science, SciELO, LILACS and open grey with these MeSH "bone remodeling", "diabetes mellitus", "orthodontic" and "tooth movement". Five articles remained after search strategy and were analyzed. In sum, no clinical studies were found, the evidence was limited to animal studies (rats). The results suggest that there are differences in bone remodeling and tooth movement during the application of orthodontic forces in animals with diabetes mellitus when compared to healthy animals, especially when the disease is associated with periodontal disease. However, the results are still controversial and may be due to different study protocols.


RESUMO Com melhorias nas exigências estéticas dentárias um número crescente de adultos está em busca de tratamento ortodôntico que, juntamente com estilo de vida atual e hábitos alimentares da população adulta, torna os ortodontistas mais propensos a encontrar pacientes com distúrbios metabólicos como a diabetes mellitus. Especula-se que o paciente diabético durante o tratamento ortodôntico pode não experimentar um processo fisiológico de cura como um paciente saudável. Portanto, o objetivo deste trabalho é apresentar uma revisão atual e contextualizada dos mecanismos pelos quais a diabetes mellitus descontrolada impacta na remodelação óssea e no movimento dentário ortodôntico durante a aplicação de forças ortodônticas. Foram pesquisadas as seguintes bases de dados MEDLINE (via PubMed), Scopus, Web of Science, SciELO, LILACS e open grey com os termos MeSH: "remodelação óssea", "diabetes mellitus", "ortodontia" e "movimento dentário". Cinco artigos permaneceram após a estratégia de busca e foram analisados. Em suma, não foram encontrados estudos clínicos, as evidências foram limitadas a estudos em animais (ratos). Os resultados sugerem que existem diferenças na remodelação óssea e na movimentação dentária durante a aplicação de forças ortodônticas em animais com diabetes mellitus quando comparados a animais saudáveis, especialmente quando a doença está associada à doença periodontal. No entanto, os resultados ainda são controversos e podem ser devido a diferentes protocolos de estudo.

ABSTRACT Orthodontics with low friction, low force, passive self-ligating brackets produces alveolar-dental remodeling, resulting in an increase in the transverse diameter of the dental arches, especially in the premolar sector. The aim of this study was to compare the modifications of the transverse diameter in the pre- and post-dental alignment cast models with orthodontics with passive self-ligating brackets in patients with moderate to severe dental discrepancy. The study included 28 patients of both sexes aged 16 to 48 years with dental discrepancies between -6 and -16 mm, treated with self-ligating Damon brackets and thermally activated Nickel- Titanium-Copper arches. With a digital pachymeter, Mitutoyo brand, five measurements were taken per dental arch: distance between canines (C), first premolars (1PM), second premolars (2 PM), first molars (1M) and second molars (2M), before and after orthodontic alignment. The variations were statistically evaluated by Student T Test for paired samples. Average distance between teeth varied with dental alignment in both jaws. The greatest increases in transverse diameter were recorded in the premolar areas. Minor though statistically significant variations also occurred in the 2M of the maxilla and in the C of the mandible In alignment with passive self-ligating brackets, there is an increase in the transverse diameter due to the development of the dental arches, mainly in the premolar sector of both jaws and also at the level of the canines in the mandible.


RESUMEN La ortodoncia con brackets autoligables pasivos, de baja fricción y baja fuerza, produce remodelación alvéolodentaria; con aumento del diámetro transversal de las arcadas, especialmente en el sector de los premolares. El objetivo de este trabajo fue comparar las modificaciones del diámetro transversal en los modelos de yeso pre y post alineación dentaria con ortodoncia con brackets autoligables pasivos en pacientes con discrepancia dentaria moderada a severa. El estudio incluyó 28 pacientes de ambos sexos de entre 16 y 48 años de edad con discrepancia dentaria entre -6 y -16mm, tratados con brackets autoligables sistema Damon y arcos termo activables de Níquel-Titanio-Cobre. Con un paquímetro digital marca Mitutoyo, se tomaron 5 medidas por arcada: distancia entre caninos (C), primeros y segundos premolares (1 PM y 2 PM) y primeros y segundos molares (1M y 2M); pre y post alineación ortodóncica. Las variaciones registradas fueron evaluadas estadísticamente mediante Student T Test para muestras apareadas. La distancia promedio entre dientes varió con la alineación dentaria en ambos maxilares. Los mayores aumentos del diámetro transversal se registraron en las zonas de los premolares. Se produjeron también variaciones menores, aunque estadísticamente significativas, en los 2M del maxilar superior y en los C del maxilar inferior. En la alineación con brackets autoligables pasivos, hay incremento del diámetro transversal por desarrollo de las arcadas, principalmente en el sector premolar de ambos maxilares y en el maxilar inferior también a nivel de los caninos.

ABSTRACT

Objetive: The objective of this clinical trial was to investigate the perception of pain during initial maxillary alignment with an adjunctive procedure of micro-osteoperforations (MOPs) compared to conventional orthodontics. Material and methods: This study design was a single-centre, two-arm parallel prospective randomised clinical trial. Thirty consecutive adult subjects (25 females and 5 males; mean age ± SD, 22.66 ± 3.27 years) with 5-8mm moderate upper labial segment crowding were randomly allocated using block randomisation into intervention and control group. All subjects had first premolar extractions, bonded conventional fixed appliances and 0.014-inch nickel-titanium archwire was placed for initial alignment. The intervention group received a 3-mm deep MOPs procedure under local anaesthesia using a Propel device (PROPEL Ortho Singapore) on the labiogingival aspect between the maxillary incisors. Both groups received a set of 100 mm visual analogue scale to complete over the first week, recording pain at 24 hours, 3 days and 1 week. Data were analysed using repeated-measures analysis of variance (ANOVA). Results: There was a statistically significant difference observed in perceived pain levels between MOPs and the control group on day 1, day 3 and day 7 postoperatively. Pain perception was significantly lower in the intervention group at all time points. Conclusion: Accelerating orthodontic tooth movement with MOPs did not accentuate pain perceived during initial maxillary alignment with fixed appliances.


Objetivo: El objetivo de este ensayo clínico fue investigar la percepción del dolor durante la alineación maxilar inicial con un procedimiento adyuvante de micro-osteoperforaciones (MOP) en comparación con la ortodoncia convencional. Material y Métodos: El diseño de este estudio fue un ensayo clínico aleatorizado prospectivo paralelo de dos brazos y un solo centro. Treinta sujetos adultos consecutivos (25 mujeres y 5 hombres; edad media ± DE, 22,66 ± 3,27 años) con apiñamiento moderado del segmento labial superior de 5-8 mm se asignaron al azar mediante la asignación al azar en bloques en el grupo de intervención y de control. A todos los sujetos se les realizaron extracciones de los primeros premolares, se colocaron aparatos fijos convencionales adheridos y se colocó un arco de níquel-titanio de 0,014 pulgadas para la alineación inicial. El grupo de intervención recibió un procedimiento de MOP de 3 mm de profundidad bajo anestesia local utilizando un dispositivo Propel (PROPEL Ortho Singapore) en la cara labial de los incisivos superiores. Ambos grupos recibieron un conjunto de escala analógica visual de 100 mm para completar durante la primera semana, registrando el dolor a las 24 horas, 3 días y 1 semana. Los datos se analizaron mediante análisis de varianza de medidas repetidas (ANOVA). Resultados: Se observó una diferencia estadísticamente significativa en los niveles de dolor percibido entre los MOP y el grupo de control el día 1, el día 3 y el día 7 del postoperatorio. La percepción del dolor fue significativamente menor en el grupo de intervención en todos los momentos. Conclusión: La aceleración del movimiento dental de ortodoncia con MOP no acentuó el dolor percibido durante la alineación maxilar inicial con aparatos fijos.
